The Speech Therapist in Home Health is responsible for the assessment and evaluation of patient care needs in treating speech and language disorders, training functionality in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Speech Pathologist determines a treatment plan, executes interventions aimed at improving and enhancing the patient's well-being, and monitors the patient's progress effectively. While performing the duties of this class, an employee is regularly required to stand, walk and sit, talk or hear both in person and by telephone, uses hands to finger, handle or feel objects or controls, reach with hands and arms. An employee also is regularly required to stoop, kneel, bend, crouch or crawl, climb or balance and lift up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, depth perception, color vision and the ability to adjust focus. Work effectively independently without close supervision; organize, set priorities, take initiative and exercise sound independent judgment within areas of responsibility; perform duties that require high attention to detail and application of rules and specific procedural requirements.
Maintain compliance with the requirements for continuing education hours and/or units as determined by the Department of Alcohol and Drug Programs and/or other regulatory agencies. Conducts searches of resident rooms as needed in accordance with MAAC policies and applicable legal requirements. Prepares exit plans, discharge summaries and certificates of completion for residents who have successfully completed the program; contacts discharged residents for follow-up. Communicates with parole officers, probation officers, advocates, and lawyers in person or by telephone to discuss residents’ status and progress. Screens program applicants for admission or referral to another treatment facility or program. You don’t have to be enrolled in VA health care or have a service-connected disability.

Assess the participants’ immediate needs and ensure participants’ receipt of needed services through personal service or making appropriate referrals. Understand, interpret and respond to client needs and expectations; explain and enforce applicable laws, codes, policies and procedures. Plans, schedules and supervises residents’ daily activities, including classes, appointments, outings, special events and community involvement. Regularly meets with residents to track progress and prepares resident progress reports. Schedules and conducts applicant interviews; intakes and discharges residents; terminates residents from program as necessary.
Any Veteran or service member, including members of the National Guard and Reserve forces, who experienced military sexual trauma is eligible to receive counseling. Develops, revises and updates individual recovery plans with residents that establish specific treatment goals. Our services are also available to family members when their participation would support the growth and goals of the Veteran or active-duty service member. We also offer bereavement services to family members of Veterans who were receiving Vet Center services at the time of the Veteran’s death, and to the families of service members who died while serving on active duty.

Prepares weekly chore lists for residents and ensures chores are complete. Logs, dispenses and disposes of prescribed medications for residents. Due to high demand for services, new clients are oriented to the Vet Center through weekly group orientation. We also have non-traditional hours that change periodically given our community’s needs. Vet Centers are community based to be more accessible in areas where you live.

Whether you come in for one-on-one counseling or to participate in a group session, at Vet Centers you can form social connections, try new things, and build a support system with people who understand you and want to help you succeed. Call our Veterans Crisis Line at 988 and press 1 to get support anytime day or night. Our Vet Center can also connect you with ongoing counseling and services. Look over, record, and report the patient's response to treatment and any alterations in the patient's condition. Interpret, plan, establish, and execute a proper treatment program in accordance with the physician's referral.
We can help connect you to VA and community resources that will support you in achieving your goals. Chula Vista Vet Center has specially trained therapists on staff to support your needs and can facilitate inpatient treatment if necessary. We offer couples and family counseling to support you as you work toward meeting your goals. Keep a record of each patient visit and incorporate detailed notes into the medical record. Make recommendations based on the home setting and individual needs of the patient. Must have and maintain a valid California Driver’s License, daily use of a personal, insured vehicle, and have and maintain a motor vehicle driving record acceptable to MAAC’s insurance carrier.
We provide private organizations and community agencies education on our Veteran community and military culture. Together, we build referral networks to expand support for Veterans, service members, and their families. Communicate effectively orally and in writing with a variety of individuals representing diverse cultures and backgrounds; understand and follow written and oral instructions; maintain confidentiality of agency documents and records. Performs case management record keeping tasks; assists in preparing required program performance reports; maintains and updates center waiting lists; prepares and types correspondence; creates resident files and applicant/resident information. We offer assessment and support through private counseling and group therapy. We can also refer you to VA or community counseling for treatment and therapy resources.
If you’re struggling with issues like PTSD, depression, grief, anger, or trauma, we offer counseling and other support. Review the patient's responses to treatment and make recommendations to the physician related to the need of continuing or discontinuing the speech therapy treatment.
